This is a remarkable opportunity to become part of a dedicated team focused on delivering a comprehensive Stroke Recovery Service. We are seeking an enthusiastic, organized, and motivated individual to support stroke survivors and their families across the region.

Position: Stroke Recovery Coordinator

Location: Home-based with extensive travel required for team meetings and other work-related engagements.

Hours: Part-time, 25 hours per week with flexible working options available.

Salary: Approximately £19,099 per annum (FTE approximately £26,700 per annum).

Contract: This is a fixed-term contract until 31 March 2025, with current funding secured for this duration.

The Role

Reporting to the Stroke Service Delivery Coach, the coordinator will collaborate with local health and social care services to manage referrals and provide support to stroke survivors and their caregivers. You will offer personalized information, guidance, and support, conducting regular reviews to ensure ongoing assistance.

Key Responsibilities:

  • Collaborate with local health and social services to manage referrals for stroke survivors and their caregivers.
  • Deliver personalized information, guidance, and support to address identified needs.
  • Conduct Needs Assessments with stroke survivors and assist them in developing a recovery plan.
  • Provide regular reviews to help individuals establish and achieve their personal goals.

About You

The ideal candidate will possess:

  • A background in caring or charity work, particularly supporting individuals with disabilities.
  • Demonstrated experience in collaborating with various organizations to achieve shared objectives.
  • Experience working with individuals who may have additional communication support needs.
  • Basic proficiency in Microsoft systems.

This role necessitates extensive travel across a broad geographical area to visit individuals in their homes and community settings. Candidates must be able to demonstrate their ability to meet this travel requirement during the application and interview process.
