Rehabilitation Assistant

We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our Outpatient and Rehabilitation Services team at Rennie Grove Peace Hospice Care. As a key member of our multi-professional team, you will play a vital role in providing holistic care to adult patients, their carers, and families.
Key Responsibilities- Work collaboratively with the rehabilitation therapy team to plan and deliver a palliative rehabilitation model of service, ensuring patients have access to interventions that enable them to reach their fullest physical, psychological, social, and vocational potential.
- Support the physiotherapist in writing and reviewing individualized care plans as required.
- Implement exercise programs as set by the physiotherapist and provide advice on progress, including exercise group provision working with volunteers.
- Assist with education and training of members of the multi-disciplinary team and community services.
- Attend relevant training courses and meetings, especially those relating to self-management and rehabilitation in palliative care.
- Participate in annual appraisal programs and adhere to Hospice policies and guidelines.
- NVQ qualified to level 3 or equivalent or level 4 or rehab/physiotherapy/occupational therapy Assistant qualification/experience.
- Experience of working in a related care setting.
- Experience of working as either a healthcare assistant, physiotherapy assistant, occupational therapy assistant, running exercise groups.
- Understanding of palliative care and Hospice philosophy.
- A good understanding of technology with a working knowledge of Microsoft office packages (e.g., Word, Excel, and Outlook).
- Able to build good working relationships.
- Able to communicate effectively both verbally and in writing.
- Good administrative and record-keeping skills and able to work with good attention to detail.
- Strong organizational skills.
- Sensitivity and understanding to the needs of volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves.
