Rehabilitation Assistant Outpatients and Rehabilitation Specialist

The purpose of this role is to provide holistic care to adult patients, their carers and families, as part of a multidisciplinary team. This includes supporting the rehabilitation team in providing clinical, administrative and health & wellbeing support to patients, carers and families.
Main Responsibilities- Work with the rehabilitation team to plan and deliver a palliative rehabilitation model of service, ensuring patients have access to interventions enabling them to reach their fullest physical, psychological social and vocational potential.
- Support the physiotherapist in writing and reviewing individualised care plans as required.
- Assist with the planning, prioritisation and management of own workload, combining a range of IPU patient and outpatient related activities delegated by the physiotherapist.
- Implement exercise programmes as set by the physiotherapist and provide advice on progress, including exercise group provision working with volunteers.
- Support the rehabilitation team with education and training of members of the multi-disciplinary team and community services.
- Attend relevant training courses and meetings, especially those relating to self-management/rehabilitation in palliative care.
- Participate in annual appraisal programme and adhere to Hospice policies and guidelines.
Rennie Grove Peace Hospice Care is committed to providing choice and ease of access to a wide range of palliative care and bereavement services across Buckinghamshire and Hertfordshire. We provide care and support for adults and children diagnosed with cancer and other life-limiting illnesses, and their families, visiting day and night.
Person Specification- Essential:
- NVQ qualified to level 3 or equivalent, or level 4 or rehab/physiotherapy/occupational therapy Assistant qualification/experience.
- Desirable:
- Qualification in the field of exercise/palliative care/physiotherapy.
