Job Description

**Job Summary:**

We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our multidisciplinary team at Central London Community Healthcare NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality patient care and support in a rehabilitation setting.

Key Responsibilities:
  • **Patient Care:**
    • Assist qualified staff in carrying out individual and group treatment plans to achieve agreed treatment objectives.
    • Provide therapy interventions as part of a multidisciplinary team to patients within an agreed scope of practice.
  • **Communication and Teamwork:**
    • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
    • Work collaboratively with the multidisciplinary team to achieve patient-centered outcomes.
  • **Professional Development:**
    • Participate in ongoing professional development to enhance skills and knowledge in rehabilitation and patient care.
    • Seek feedback and guidance from qualified staff to improve practice.
  • **Administrative Tasks:**
    • Accurately document patient information and treatment plans.
    • Contribute to the maintenance of a clean and safe working environment.
Person Specification:
  • **Essential Criteria:**
    • Good general standard of education at GCSE level (Grade 9-4) including English and Mathematics or equivalent Level 2 qualification(s).
    • NVQ in a relevant subject or equivalent skills gained through work experience.
    • Previous work experience in a hospital or community rehabilitation setting.
    • Experience working with older people.
    • Team working skills.
  • **Desirable Criteria:**
    • Willingness to undertake an NVQ or an equivalent qualification.
    • Keen to gain professional qualification in a relevant area.
    • Previous experience as a healthcare or therapy assistant.
    • Experience working as part of a multi-professional team.
Skills and Knowledge:
  • **Essential Criteria:**
    • Basic knowledge and understanding of the conditions treated in the rehabilitation setting.
    • Good range of skills in a variety of practical areas.
    • Effective written and oral communication skills.
    • IT literate.
    • Ability to manage own workload and to prioritize effectively.
    • Ability to work in scope of own knowledge/ability.
    • Demonstrate competence in teaching practical skills to clients.
    • Time management skills.
    • Interest in gaining new skills and developing knowledge.
    • Understanding the principles of professional conduct.

    • Knowledge of the role and scope of members of the multidisciplinary team.
    • Ability to work effectively under pressure.
