Rehabilitation Assistant Outpatients and Rehabilitation Specialist

The purpose of this role is to provide holistic care to adult patients, their carers, and families as part of a multidisciplinary team. This includes supporting the rehabilitation team in delivering clinical, administrative, and health & wellbeing support to patients, carers, and families.
Main Responsibilities- Work with the rehabilitation therapy team to plan and deliver a palliative rehabilitation model of service, ensuring patients have access to interventions enabling them to reach their fullest potential.
- Support the physiotherapist in writing and reviewing individualised care plans as required.
- Contribute to the development of relationships with relevant members of the multidisciplinary community team.
- Attend relevant training courses and meetings, especially those relating to self-management/rehabilitation in palliative care.
- Participate in annual appraisal programme and adhere to Hospice policies and guidelines.

Person Specification- Essential:
- NVQ qualified to level 3 or equivalent or level 4 or rehab/physiotherapy/occupational therapy Assistant qualification/experience.
- Experience of working in a related care setting.
- Experience of working as either a healthcare assistant, physiotherapy assistant, occupational therapy assistant, running exercise groups.
- Understanding of palliative care and Hospice philosophy.
- A good understanding of technology with a working knowledge of Microsoft office packages ie Word, Excel and Outlook.
- Able to build good working relationships.
- Able to communicate effectively both verbally and in writing.
- Good administrative and record keeping skills and able to work with good attention to detail.
- Strong organisational skills.
- Sensitivity and understanding to the needs of the volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves.
- Desirable:
- Experience of being part of a multidisciplinary team.
- Experience working within palliative care.
- Experience of working with and leading & motivating teams of volunteers.
- Strong problem-solving skills and able to implement new initiatives.
- Health & Safety Training.
