We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our Outpatient and Rehabilitation team at Rennie Grove Peace Hospice Care. As a key member of our multi-professional team, you will play a vital role in providing holistic care to adult patients, their carers, and families.
Key Responsibilities- Work collaboratively with the physiotherapist to plan and deliver a palliative rehabilitation model of service, ensuring patients have access to interventions enabling them to reach their fullest potential.
- Support the physiotherapist in writing and reviewing individualized care plans as required.
- Implement exercise programs as set by the physiotherapist and provide advice on progress, including exercise group provision working with volunteers.
- Assist the rehabilitation team with education and training of members of the multi-disciplinary team and community services.
- Attend relevant training courses and meetings, especially those relating to self-management and rehabilitation in palliative care.
- NVQ qualified to level 3 or equivalent, or level 4, or rehabilitation/physiotherapy/occupational therapy assistant qualification/experience.
- Experience of working in a related care setting, including experience as a healthcare assistant, physiotherapy assistant, occupational therapy assistant, or running exercise groups.
- Understanding of palliative care and hospice philosophy.
- Good understanding of technology, including Microsoft Office packages (Word, Excel, Outlook).
- Able to build good working relationships and communicate effectively both verbally and in writing.
- Good administrative and record-keeping skills, with attention to detail.
- Strong organizational skills and sensitivity to the needs of volunteers and customers.
