Key Responsibilities
  • Collaborate with the Nurse Manager to oversee the care of a small caseload of clients with diverse complex care needs.
  • Induct and train Care Assistants and Support Workers to ensure they possess the necessary clinical skills and knowledge to meet clients' needs.
  • Maintain regular shifts within your caseload to uphold familiarity with each client's requirements and provide consistent, personalised care.
  • Work with the Coordination Team to maintain 100% coverage of clients' care packages and identify solutions for absence cover.
  • Mentor support workers, conduct observations, and identify training needs.
  • Monitor client satisfaction, escalate concerns, and produce monthly reports for effective service management.
Requirements
  • Hold a Level 2 Diploma in Health and Social Care.
  • Commit to attaining a Level 3 Diploma in Health and Social Care within two years of joining (funded by Superior Healthcare).
  • Previous experience working with people who have disabilities and complex health needs.
