Field Care Supervisor Job Description

Job Summary:

We are seeking a skilled and compassionate Field Care Supervisor to join our dynamic complex care team in East Kent. As a Field Care Supervisor, you will play a vital role in overseeing the care of clients with diverse complex care needs, ensuring they receive exceptional service from a well-trained and enthusiastic team.

Key Responsibilities:

  • Work closely with the Nurse Manager to set up new care packages and ensure seamless transitions for clients.
  • Maintain regular shifts within your client caseload, upholding familiarity with each client's requirements and providing consistent, personalized care per their care plan.
  • Collaborate with the Nurse Manager to review and update clients' care plans, identifying areas for improvement and implementing changes as needed.
  • Identify and communicate upcoming staffing gaps, engage in recruitment, and introduce new hires to clients, ensuring a smooth onboarding process.
  • Mentor support workers, conduct observations, and identify training needs to ensure the team is equipped to provide high-quality care.
  • Maintain timely and accurate records of care and supervisory activities, ensuring compliance with regulatory requirements.
  • Act in the best interest of clients, prioritizing their needs and well-being above all else.
  • Monitor client satisfaction, escalate concerns, and produce monthly reports to inform care delivery and improvement initiatives.
  • Attend mandatory paid training, including achieving a QCF Level 3 within two years of employment, to enhance your skills and knowledge.

Requirements:

  • Hold a Level 2 Diploma in Health and Social Care, with a commitment to attaining a Level 3 Diploma within two years of joining (funded by Superior Healthcare).
  • Previous experience working with people who have disabilities and complex health needs, such as those with tracheostomies, ventilators, gastrostomy tubes, and bowel management care.
