Job Title: Qualified/Senior Social Worker - Older People

About the Role:

We are seeking a highly skilled and compassionate Social Worker to join our Older People and Disabilities Team at Randstad Care. As a Social Worker in this team, you will play a vital role in supporting adults with complex needs, particularly older individuals and those with physical disabilities.

Key Responsibilities:

  • Assess the needs of adults with complex needs, particularly older individuals and those with physical disabilities.
  • Develop and implement personalized care plans to enhance independence, well-being, and quality of life.
  • Coordinate access to services that support adults with complex needs, including healthcare, social care, and community resources.
  • Work collaboratively with other professionals, including healthcare providers, social care teams, and community organizations, to ensure seamless support for adults with complex needs.
  • Provide emotional support and guidance to adults with complex needs and their families, promoting independence and well-being.

Requirements:

  • Qualified Social Worker with a degree in Social Work or a related field.
  • Experience working with adults with complex needs, particularly older individuals and those with physical disabilities.
  • Strong assessment and care planning skills, with the ability to develop and implement personalized care plans.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with other professionals and stakeholders.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
