About the Role:
We are seeking a highly skilled and experienced Senior Social Worker to join our Adult Locality Team. As a Senior Social Worker, you will be responsible for undertaking strengths-based assessments, reviews, and support planning to enable residents to maximize their independence, improve quality of life, and build their resilience.
Key Responsibilities:
- Undertake strengths-based assessments, reviews, and support planning to enable residents to maximize their independence, improve quality of life, and build their resilience.
- Assist in arranging short-term care and provide clients and carers with respite to reduce carer breakdown.
- Work with adult clients, relatives, carers, and communities to help them make informed decisions, clarifying and expressing their needs, and contributing to support planning.
- Liaise with colleagues across the Haringey health and social care partnership to gather information relevant to assessment and support planning activities.
- Undertake reviews of care packages.
- Attend safeguarding strategy meetings when required.
- Complete Continuing Health Care checklists and identify those that require full assessments.
Requirements:
- QSW Status - including qualification, Social Work England registration, and completed ASYE.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong assessment and planning skills.
- Knowledge of adult social care policies and procedures.
