About the Role

We are seeking a highly skilled and motivated Occupational Therapist to join our Adult Social Care and Health team in the High Peak area of Derbyshire. As an Occupational Therapist, you will play a vital role in supporting adults to maximise their independence and well-being.

Key Responsibilities
  • Assess and develop person-centred plans to support adults with physical, learning disabilities, and mental health conditions.
  • Work collaboratively with multi-disciplinary teams to deliver a proportionate, personalised, safe, and sustainable response for local adults, their carers, and families.
  • Develop and implement equipment and adaptation solutions to support adults with daily living activities.
  • Manage and prioritise casework, ensuring timely and effective interventions.
  • Build and maintain positive relationships with partner professionals and agencies.

Requirements
  • Registration with the HCPC or application for registration.
  • Knowledge and/or experience of working within a person-centred, holistic, and strengths-based approach.
  • Assessment and development of person-centred plans.
  • Knowledge and/or experience of physical, learning disabilities, and mental health conditions and their impact on occupational performance.
  • Equipment and adaptation solutions.
  • Good communication and interpersonal skills.
  • Ability to manage and prioritise casework.
