Position Overview
We are looking for a Senior Community Therapy Assistant who embodies a compassionate, patient-focused ethos and is dedicated to enhancing community health services.
The successful candidate will join our community therapy unit, which encompasses a swift response initiative. Our collaborative, multi-disciplinary teams deliver prompt, personalized interventions that are tailored to support patients in their residences and local care facilities.
Our mission is to avert unnecessary hospital admissions and ensure timely discharges from both acute and community care settings, enabling individuals to maintain their independence in their own homes for as long as possible. We emphasize a holistic approach to patient care, providing therapy services directly in their homes, equipped with necessary tools to facilitate this.
You will receive robust support from a proficient, multi-disciplinary team, which includes Physiotherapists, Occupational Therapists, Nurses, Support Workers, and the Acute Home Visiting Service. We also maintain close collaboration with our Adult Care services.
Key Responsibilities
Your primary duties will include:
- Delivering and assisting in the provision of comprehensive, high-quality care.
- Operating within a suitable supervisory and monitoring framework, adhering to assessment protocols, established multi-disciplinary treatment plans, and managing ongoing treatment, education, and guidance for a designated patient caseload.
- Implementing, adjusting, and assessing treatment programs independently.
- Conducting rapid interventions, initial assessments, and patient reviews, while developing specific, patient-centered care plans to achieve set objectives and coordinating with other specialized services within the organization.
- Organizing and prioritizing daily work tasks and responsibilities, demonstrating the ability to work autonomously while complying with organizational policies, and delegating tasks to Therapy Support Worker colleagues as necessary.
Travel Requirements
This role necessitates extensive travel and the transportation of equipment throughout the designated locality. Candidates must have access to a vehicle suitable for work-related travel or be able to meet the service's travel requirements.

Qualifications & Training
Essential:
- Willingness to pursue further recognized training relevant to the role.
- Good standard of general education, including English Language and Mathematics GCSE or equivalent.
- NVQ Level 4 in Health and Social Care, Foundation degree, or relevant NHS experience, along with a commitment to complete necessary studies.
Desirable:
- Assistant Practitioner Qualification or equivalent.
Experience & Knowledge
Essential:
- Experience in multi-disciplinary team or integrated team settings.
- Experience working with patients presenting complex multi-pathologies.
- Familiarity with recognized falls prevention techniques, including outcome measures.
- Understanding of factors influencing health and well-being.
Desirable:
- Experience in a community-based team.
- Working knowledge of clinical audit and service enhancement.
Skills & Attributes
