About the Role

We are seeking a skilled and compassionate Community Response Support Worker to join our team at Derbyshire Community Health Services NHSFT. As a key member of our Community Response Team, you will play a vital role in supporting individuals in their own homes, promoting independence, and enhancing their overall wellbeing.

Key Responsibilities
  • Provide high-quality care and support to patients in their own homes, working collaboratively with clinicians and other healthcare professionals.
  • Assist patients in achieving their goals and optimizing their health and independence.
  • Document patient interactions and progress, maintaining accurate and up-to-date records.
  • Travel across the county to attend essential training and other required training for your role.
Requirements
  • NVQ Level III in care or equivalent qualification.
  • Experience of working in a healthcare setting.
  • Good verbal communication skills and well-developed interpersonal skills.
  • Able to keep accurate records and travel across the county.
