Job Title: Dementia Support Worker - Community Specialist

We are seeking a highly skilled and compassionate Dementia Support Worker to join our team at Derbyshire Healthcare NHS Foundation Trust. As a Dementia Support Worker, you will play a vital role in providing support and education to individuals with dementia and their families.

Job Summary:

The successful candidate will work in partnership with the High Peak and North Dales Community Mental Health Team (CMHT) to provide practice-based support for older adults with mental health issues related to cognitive impairment and dementia. You will work under the supervision of registered professionals from within the CMHT and the Older Peoples Community Mental Health Team.

Main Responsibilities:
  • Provide support and education to individuals with dementia and their families
  • Work in partnership with the CMHT to provide practice-based support
  • Attend team meetings, clinical MDMs, and access supervision and training
  • Support individuals presenting with cognitive issues and with a diagnosed dementia
  • Offer education around dementia, signposting to appropriate support services, and advice around 'living well with dementia'
Requirements:
  • Level III NVQ in Health/Social Care or equivalent
  • Good communication skills
  • Evidence of working as part of a team
  • Ability to work on own initiative and be aware of own limitations
  • Basic IT skills
  • Able to meet the travel requirements of the post
