Job Overview

We are seeking a highly skilled and compassionate Dementia Support Worker to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our Community Mental Health Team, you will play a vital role in providing support and education to individuals with dementia and their families.

Main Responsibilities
  • Provide practice-based support for Older Adults with Mental Health issues related to cognitive impairment and Dementia.
  • Work in partnership with the High Peak Primary Care Network to deliver high-quality services.
  • Offer education and guidance on dementia, signposting to relevant support services, and completing pre-assessment support.
  • Collaborate with registered professionals from within the Primary Care Network and the Older Peoples Community Mental Health Team.
  • Travel to various GP surgeries within the High Peak area to provide support and services.
Requirements
  • Level III NVQ in Health/Social Care or equivalent.
  • Good communication skills and ability to work as part of a team.
  • Basic IT skills and ability to meet travel requirements of the post.
  • Demonstrate knowledge of dementia and experience of working with individuals with dementia.
