Job Title: Community Response Support Worker

We are seeking a highly motivated and compassionate Community Response Support Worker to join our team at Derbyshire Community Health Services NHS Foundation Trust. As a Community Response Support Worker, you will play a vital role in providing high-quality care and support to patients in their own homes.

About the Role

The Community Response Team provides short-term support to patients who have been discharged from hospital or require assistance with daily living tasks. As a Community Response Support Worker, you will work closely with patients, families, and healthcare professionals to develop personalized care plans that promote independence and well-being.

Key Responsibilities
  • Provide high-quality care and support to patients in their own homes
  • Develop and implement personalized care plans in collaboration with patients, families, and healthcare professionals
  • Assist patients with daily living tasks, such as bathing, dressing, and medication management
  • Work closely with the multidisciplinary team to ensure seamless care coordination
  • Maintain accurate records and reports to ensure high-quality care and compliance with regulatory requirements
Requirements

To be successful in this role, you will need:

  • NVQ Level III in care or equivalent
  • Experience working in a healthcare setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a variety of shifts, including evenings and weekends
