Job Summary:

We are seeking a highly skilled and experienced Occupational Therapist to join our team at Derbyshire County Council Adult Social Care and Health. As an Occupational Therapist, you will play a vital role in supporting adults to maximise their independence and well-being.

Key Responsibilities:

  • Assess and develop person-centred plans to support adults with physical, learning disabilities, and mental health conditions.
  • Work collaboratively with a multi-disciplinary team to deliver a proportionate, personalised, safe, and sustainable response for local adults, their carers, and families.
  • Develop and implement equipment and adaptation solutions to support adults to maintain their independence.
  • Manage and prioritise casework, ensuring timely and effective interventions.
  • Build and maintain positive relationships with a wide range of partner professionals and agencies.
  • Stay up-to-date with relevant legislation, policies, and best practices in adult social care and health.

Requirements:

  • Registration with the Health and Care Professions Council (HCPC) or eligibility for registration.
  • Knowledge and experience of working within a person-centred, holistic, and strengths-based approach.
  • Experience of assessing and developing plans to support adults with physical, learning disabilities, and mental health conditions.
  • Knowledge and experience of equipment and adaptation solutions.
  • Ability to manage and prioritise casework, ensuring timely and effective interventions.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a multi-disciplinary team.
