About This Role
We are seeking a highly motivated and experienced Diabetes Care Specialist Nurse to join our dedicated team at West Suffolk NHS Foundation Trust. In this role, you will play a vital part in providing comprehensive care to patients with diabetes under the guidance of Consultant-led services.
Your Responsibilities
- Conduct Nurse-Led Clinics and Insulin Start Groups, delivering structured education programs tailored to individual patient needs.
- Provide ongoing support and follow-up care for patients with diabetes, ensuring their well-being and adherence to treatment plans.
- Collaborate effectively with multidisciplinary teams to ensure seamless and coordinated patient care.
