Position Overview

Primary Care Careers is excited to announce an opportunity for a skilled healthcare practice nurse to become a vital part of our dedicated team. This role offers flexibility with options for full-time or part-time engagement, tailored to the right candidate's preferences.

Key Responsibilities

  • Conducting comprehensive health evaluations, administering medications, and performing essential diagnostic procedures.
  • Delivering and assessing a variety of health interventions to meet the holistic health and social care requirements of patients.
  • Promoting general health and well-being while developing personalized, patient-centered care plans for individuals with or at risk of chronic conditions.
  • Managing and treating chronic health issues in accordance with established national and local guidelines, as well as the specific needs of the practice.
  • Reviewing patient medications for effectiveness based on evidence-based practices and collaborating with patients to ensure adherence to treatment plans.
  • Prioritizing health concerns and providing timely interventions to assist patients in complex or urgent situations, including initiating emergency care when necessary.
  • Implementing and participating in vaccination and immunization initiatives for both adult and pediatric populations, including travelers.
  • Addressing the needs of patients requiring opportunistic wound care.
  • Conducting hypertension assessments and management.
  • Facilitating asthma, COPD, and diabetes management clinics and evaluations.
  • Performing new patient assessments and health checks.

Candidate Profile

Required Experience

  • Background in primary care (preferred but not mandatory).
  • Experience as a practice or community nurse (preferred but not mandatory).
  • Proficiency in chronic disease management.
  • Skills in wound care, ECGs, venepuncture, and immunizations, along with handling new patient registrations and pathology requests.
  • Capability to maintain accurate clinical documentation.
  • Strong interpersonal communication skills.
  • Ability to adhere to clinical protocols.
  • Competence in IT systems.

Essential Qualifications

  • Current NMC registration with an active PIN.
