About the Role

Elysium Healthcare is seeking a highly skilled and experienced Registered Nurse to join our team as a Physical Health Nurse Specialist. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and promoting the physical health and well-being of our service users.

Key Responsibilities

  • Develop and implement individualized physical health care plans to meet the unique needs of each service user.
  • Conduct regular health assessments and monitor the physical health of service users, identifying any potential health risks or concerns.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for service users.
  • Provide education and training to service users and their families on healthy living, disease prevention, and management of chronic conditions.
  • Lead and participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving health outcomes.
  • Contribute to the development and implementation of policies and procedures related to physical health care.
  • Participate in quality improvement initiatives and contribute to the evaluation of physical health care services.

Requirements

  • Registered Nurse with a valid NMC registration.
  • Minimum of 3 years post-registration experience in a physical health care setting.
  • Experience in NG feeding and phlebotomy.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
