About the Role
We are seeking an experienced and motivated Occupational Therapist to join our Integrated Community Team in Thurrock. As a key member of our team, you will be responsible for providing high-quality patient-centered care to individuals with physical conditions, including neurological, orthopaedic, and musculoskeletal disorders.
Key Responsibilities
- Assess and develop rehabilitation plans for patients with complex needs
- Work collaboratively with community nurses, community matrons, physiotherapists, and other healthcare professionals to ensure seamless care delivery
- Provide education and training to patients, families, and caregivers on self-care and independence skills
- Contribute to the development and implementation of service improvement initiatives
Requirements
- Experience of working at B5/B6 level in a community rehabilitation setting
- Previous NHS experience, preferably in occupational therapy
- HCPC registration and a degree in Occupational Therapy or equivalent
