Position Overview

Are you a dedicated Senior Clinical Practitioner seeking to contribute to a dynamic social enterprise that values its workforce? Primary Care 24 (PC24) is pleased to announce an opening for a Senior Clinical Practitioner within our Out of Hours Services.

The successful candidate will collaborate effectively with a diverse multi-disciplinary team to convey clinical judgments and develop tailored management strategies in partnership with system stakeholders.

Within your professional scope, you will evaluate, diagnose, and offer guidance, care, and management to patients presenting with undiagnosed or ambiguous health issues through both telephonic consultations and in-person assessments for patients requiring home visits.

In this role, you will exhibit clinical reasoning, critical analysis, and independent practice, with support from GP colleagues in the out-of-hours environment.

Key Responsibilities

  • Conduct in-person and telephonic triage consultations, showcasing advanced skills in clinical evaluation and diagnosis.
  • Analyze pathology and radiology results and take appropriate actions as necessary.
  • Demonstrate exceptional interpersonal and communication abilities to facilitate meaningful discussions with patients and their families.
  • Incorporate core values and expectations of care, compassion, courage, communication, commitment, and competence into daily practice.
  • Prescribe and assess medications for therapeutic effectiveness, tailored to patient needs and in alignment with evidence-based practices and established protocols.
  • Design, implement, and assess an effective management plan for patients, ensuring safe discharge, referral, and/or transfer to suitable healthcare services.

Candidate Profile

The ideal candidate will be an experienced Senior Clinical Practitioner capable of delivering high-quality, individualized care to patients with a significant degree of autonomy, grounded in the four pillars of clinical practice, leadership and management, education, and research.

You will be expected to leverage your expertise in advanced clinical assessment and diagnostic reasoning to make informed decisions regarding treatment modifications. A flexible, compassionate, and proactive approach is essential for this role.

A master's degree in advanced clinical practice is required, along with the ability to perform comprehensive assessments, diagnose, plan, and prescribe medications. Experience in acute care, urgent/emergency services, or out-of-hours settings at a senior level is essential, along with the ability to conduct thorough clinical evaluations through both telephonic consultations and in-person reviews.
