Position Overview
iGPc Primary Care Network is seeking to appoint a Senior Clinical Practitioner (Nurse/Paramedic/Pharmacist) with a non-medical prescribing qualification to enhance our Acute Care Hub team. This role can be offered on a full-time or significant part-time basis.
The successful candidate will operate autonomously, conducting thorough history taking, clinical evaluations, and diagnoses within their scope of practice. You will implement a variety of care interventions, including prescribing for patients with undiagnosed conditions and supporting them as part of a collaborative care plan. Home visits and participation in practice meetings will also be part of your responsibilities.
You will be joining a dedicated team of four Advanced Nurse Practitioners/Paramedics, providing immediate access appointments for the patient population served by the PCN.
Key Responsibilities
The ideal candidate will be an experienced Senior Clinical Practitioner (Nurse/Paramedic/Pharmacist) with a non-medical prescribing qualification. Acting within their professional scope, they will deliver care to patients from initial history taking through clinical assessment, diagnosis, treatment, and evaluation of care. Responsibilities include:
- Providing direct access to an ACP for the general practice population via telephone and home visits.
- Making independent clinical decisions for which they are accountable.
- Conducting clinics for patients with undiagnosed issues, utilizing skills in history taking, physical examination, and clinical decision-making to formulate a diagnosis and management plan.
- Initiating necessary diagnostic tests and interpreting findings.
- Prescribing safe and effective medication as an independent prescriber.
- Delivering evidence-based, individualized patient care.
- Offering holistic support to patients and their families, focusing on prevention and self-care.
- Facilitating health promotion and education initiatives.
- Collaborating with multi-disciplinary teams to ensure integrated care pathways.
- Contributing to quality targets to maintain high standards of patient care.
- Identifying community health needs and developing strategies to address them.
- Participating in the development of new patient services and improving existing ones.
Teaching and Mentoring
The post-holder will:
- Foster a learning environment for patients and healthcare professionals.
- Assist in planning and delivering educational sessions for staff, including medical students and healthcare assistants.
Professional Development
The post-holder will:
- Promote evidence-based practice through research and guidelines.
- Monitor their clinical practice effectiveness through audits and peer reviews.
- Maintain professional registration and engage in continuous professional development.
- Adhere to relevant professional conduct codes.
- Document accurate consultation data in patient records.
- Stay informed about health-related policies and collaborate with the practice team on implementation strategies.
- Demonstrate leadership and advocate for the ACP role within the PCN.
Health and Safety Compliance
The post-holder must adhere to the Practice's Health and Safety policies and report incidents as per the organization's protocols.

Special Working Conditions
The post-holder may be required to travel independently for home visits and attend meetings hosted by other agencies.
Qualifications and Experience
Essential qualifications include:
- Registered General Nurse or Allied Health Professional.
- Recognized Advanced Practice qualification at master’s level or equivalent.
- Experience as an Independent Prescriber.
- Current UK driving license and insurance.
Desirable qualifications include teaching or mentoring experience.
Essential experience includes a minimum of five years post-registration experience and evidence of continuing professional development.
Desirable experience includes leadership roles and telephone triage experience.
