Job Title: Advanced Clinical Practitioner - Primary Care Services

Job Summary:

We are seeking an experienced Advanced Clinical Practitioner to join our team in Primary Care Services. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality nursing services to patients across Merseyside and surrounding areas.

Key Responsibilities:

  • Deliver evidence-based nursing services to patients, working autonomously and as part of the multidisciplinary team.
  • Prescribe medication in accordance with PGD and within own scope of practice.
  • Review medication for effectiveness, need, and national guidance.
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in the development of individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.

Requirements:

  • Experience as an Advanced Clinical Practitioner in Primary Care Services.
  • Ability to prescribe medication in accordance with PGD and within own scope of practice.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
