Job Title: Adult Advanced Practitioner

Service Care Solutions Ltd is seeking a highly skilled and experienced Adult Advanced Practitioner to join our team. As a key member of our Primary Care Service, you will be responsible for delivering evidence-based nursing services to patients across Merseyside and surrounding areas.

Key Responsibilities:
  • Deliver high-quality nursing care to patients, working autonomously and as part of a multi-disciplinary team.
  • Prescribe medication in accordance with PGD and within your scope of practice.
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in the development of individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of developing long-term conditions.
Requirements:
  • Registered Nurse with a valid NMC registration.
  • Experience in Primary Care or a related field.
  • Strong clinical skills and knowledge of nursing practices.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
