Job Summary:

The Advanced Clinical Practitioner will be responsible for assessing, planning, evaluating, treating, and discharging service users from Community Mental Health Team (CMHT) care as appropriate to their needs. This will involve liaising with other services as required, referring for medical intervention when necessary, and making changes to clinical practices based on evidence, policy, and the identified needs of service users.

Key Responsibilities:

  • Assess service users' needs and develop individualized care plans
  • Provide expert mental and physical health professional skills to provide assessment, diagnosis, intervention, and clinical guidance as appropriate
  • Liaise with other services to ensure comprehensive care and support
  • Refer service users for medical intervention when necessary
  • Make changes to clinical practices based on evidence, policy, and service user needs
  • Work as an advanced clinical lead to provide guidance and support to the team

Requirements:

  • Advanced clinical skills and knowledge in mental and physical health
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to assess and manage complex cases
  • Knowledge of relevant policies and procedures
