Job Title: Homeless Outreach Specialist

Job Summary: We are seeking a highly motivated and compassionate Homeless Outreach Specialist to join our team at TWO SAINTS. As a key member of our outreach team, you will provide personalized support to individuals experiencing homelessness, helping them access essential services and find stable housing.

Main Responsibilities:

  • Provide assertive outreach to long-term rough sleepers, maintaining a consistent and persistent approach to support their needs.
  • Engage with rough sleepers on the street, working closely with homeless healthcare and street homeless prevention teams.
  • Manage a caseload of clients, maintaining accurate records of support offered, actions to be completed, and outcomes achieved.
  • Conduct initial assessments to identify client needs and develop person-centered outcome and holistic support plans.
  • Identify positive actions and small successes to create lasting change.
  • Develop dynamic risk assessments with a flexible and creative approach.
  • Provide information and support on housing, navigating pathways, and advocating for clients.
  • Liaise with statutory and voluntary agencies on behalf of clients, including Homeless Day Centre.
  • Accompany clients to interviews and appointments, providing emotional support in crisis situations.
  • Support clients with practical skills development, such as managing UC claims, shopping, probation, and health appointments.
  • Support clients to maintain a safe environment and avoid eviction if accommodated.
  • Attend community meetings and presentations with managers to support additional funding for the role.
  • Access personalization funds creatively to identify opportunities and achieve outcomes.
  • Support the team and participate in reflective practice.
  • Develop and maintain excellent relations with the local community.

Requirements:

  • Clear verbal and written English
  • Good IT and keyboard skills
  • Polite, assertive manner, ability to self-motivate
  • Passion for working in a client/customer-facing role
  • Knowledge of the benefits system and tenancy legislation
  • Interest and genuine concern for homelessness and related issues
  • Knowledge of voluntary and statutory agencies
