About the Role

We are seeking a highly motivated and compassionate Homeless Outreach Support Specialist to join our team at Two Saints. As a key member of our outreach team, you will play a vital role in supporting individuals who are sleeping rough, helping them to access essential services and accommodation.

Key Responsibilities
  • Provide Intensive Support: Offer personalized support to long-term rough sleepers, maintaining a consistent and persistent approach to help them achieve their goals.
  • Engage with Rough Sleepers: Work alongside homeless healthcare and street homeless prevention teams to engage with rough sleepers on the street, providing support and guidance.
  • Manage Client Caseload: Maintain accurate records of support offered, actions to be completed, and outcomes achieved for each client.
  • Assess Client Needs: Conduct initial assessments to identify client needs and develop person-centered outcome and holistic support plans.
  • Identify Positive Actions: Help clients identify positive actions and small successes to create lasting change.
  • Dynamic Risk Assessments: Create flexible and creative risk assessments to ensure client safety.
  • Information and Support: Provide information and support on housing pathways, advocating for clients and liaising with statutory and voluntary agencies.
  • Accompany Clients: Accompany clients to interviews and appointments, providing emotional support in crisis situations.
  • Practical Skills Development: Support clients in developing practical skills, such as managing UC claims, shopping, and health appointments.
  • Community Engagement: Develop and maintain excellent relations with the local community, attending community meetings and presentations as required.
Requirements
  • Clear Verbal and Written English
  • Good IT and Keyboard Skills
  • Polite, Assertive Manner
  • Passion for Working with Homeless Individuals
  • Knowledge of Benefits System and Tenancy Legislation
  • Full Driver's License and Access to a Vehicle
