Main Responsibilities

The Advanced Psychosis Rehabilitation Specialist will play a crucial role in assisting individuals experiencing severe and persistent mental health challenges throughout both inpatient and community settings. The objective of this service is to deliver rehabilitation and recovery-oriented support that empowers individuals to create, develop, and lead fulfilling, independent lives.

Key responsibilities include:

  1. Providing clinical support within Community Mental Health Teams and Primary Care Networks, focusing on rehabilitation strategies that prevent skill deterioration and offer timely assistance.
  2. Acting as a navigator for clients, facilitating assessments, interventions, and referrals to suitable community resources.
  3. Fostering professional relationships with community mental health teams, rehabilitation services, and local providers to enhance service delivery.
  4. Supporting the development of community engagement initiatives aimed at reducing social isolation and promoting active participation.

Role Purpose

The primary aim of this position is to support individuals with complex mental health needs through a comprehensive rehabilitation framework. The Advanced Psychosis Rehabilitation Specialist will be responsible for:

  1. Developing a collaborative network with community mental health teams and other stakeholders to facilitate effective signposting.
  2. Managing a defined caseload, ensuring accountability for assessment, planning, implementation, and evaluation of rehabilitation interventions.
  3. Providing adaptable support to enable clients to transition smoothly within the rehabilitation pathway.
  4. Championing principles of social recovery and promoting the ethos of rehabilitation services.
