TO APPLY VISIT
**Job Purpose**
To work with with people with learning disabilities and their families/informal carers within North Wales communities. To encourage adults with a learning disability and/or autism to get their voices heard and to promote self-advocacy.
**Key Tasks**
Make and maintain contact with people with learning disabilities and their families / informal carers within the Conwy and Denbighshire area within an agreed timeframe.
Help to clarify and manage peoples’ expectations and concerns around the impact of change within their lifestyle.
Encourage and support each person to engage - Identify, modify or design a set of tools that will enable them to talk about the things that are important to (and for) their family member.
Organise and coordinate engagement discussions using the appropriate tools to record what people say.
Share with professionals the information gathered and recorded using the toolkits.
Regularly report back updates and any significant concerns Represent families in multiple evaluation processes.
Attend regular team meetings and appraisal/supervision sessions. Provide monthly Trustee reports.
General administration tasks.
**Skills, Experience and Attributes you will need**
**Essential**
A positive and motivated approach to working.
An understanding of the lived experiences of people who have a learning disability and their families.
Minimum of two years' administration experience, including the use of computer programmes such as Word, Outlook, Excel, Teams, PowerPoint, Zoom.
Great engagement skills - able to build good relationships and clearly communicate information successfully with a wide range of people. Experience of monitoring, evaluating and reporting information.
Customer service skills - to have an approachable manner and able to manage people's expectations and concerns and offer clarity, patience and support during change.
Excellent organisational skills - able to work well independently, and successfully manage a wide range of workload/caseload.
Experience of successful partnership working.
**Desirable**
Experience of project management with proven successful outcomes. Experience of working in a health and or domiciliary care setting, including with people with a learning disability.
Experience of tendering and evaluation working.
The ability to speak Welsh fluently.
**Other Requirements**
A driving licence
Access to a vehicle and ability to travel regularly across Conwy and
Denbighshire (travel costs included).
Flexible working hours (may include occasional evening working). Enhanced DBS check.
