The Complex Assessment team is responsible for assessing and tracking patients requiring a discharge to assess (D2A) bed based service within a timely manner in a non-acute setting.

The post holder will based within MPFT commissioned beds within a discharge to assess setting.

Working closely with the mulit-disciplinary team and external agencies to ensure a robust assessment and a safe discharge.

The post holder will be based within MPFT commissioned beds in a discharge to assess (D2A) setting. Working closely with multi-disciplinary teams under the direction of the Patient Flow Team. Gathering and recording information to enable accurate patient assessments to facilitate timely discharge. The post holder will be responsible for the clerical coordination of the discharge process from the point of admission until discharge whilst building robust relationships with patient’s families and the multi-disciplinary team.The post holder will be required to effectively provide support to assist in the delivery of safe and effective patient transfer and tracking within D2A Services whilst gathering and collating collateral histories and information regarding current care needs.The Discharge Facilitator will attend meetings, sharing and recording information, escalating any blocks or issues. The post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives whilst working under the supervision of the Flow Team. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence. The role will involve close working with health and social care staff and an understanding of community Home First services alongside knowledge of the Care Act 2014 and the principles of (D2A) would be desirable

KEY RESPONSIBILITIES
1. To effectively track the patients journey from admission to discharge in a timely manner using in house IT system CDS
1. To attend Multi-Disciplinary/ Best Interest meetings to track and update patient journeys which may involve some travelling across the D2A footprint to assist with the flow of patients through the service.
1. Follow-up any outstanding professional actions identified in the Multi
- Disciplinary Team meeting/ with the appropriate professional/services and establish timescale for completion ensuring CDS is kept accurate.
1. Liaison with families and professionals including external agencies to provide timely updates and discharges including the booking of transport.
2. Monitoring the progress of ongoing referrals and assessments escalating any issues to the Patient Flow Coordinator
3. Reporting of planned discharges to internal teams to facilitate timely admissions and discharges from acute settings
1. Develop and manage strong relationships with all partners and colleagues to support timely updates and flow through discharge to assess beds.
2. To be able to work closely and at pace, assisting other members of the team to meet deadlines, sharing workload and providing cover for absences where necessary, attending meetings and updating CDS.
3. Flexible approach - demonstrating ability to respond positively to changes in allocation of work at short notice to set deadlines.

Systems and equipment
1. Update and maintain accurate information within shared local health economy patient tracking systems.
2. Ensures data and reporting information is presented in a timely manner as required by keeping CDS system updated, adding and discharging patients, uploading documents, ensuring data system is accurate for reporting needs.
