Discharge Facilitator

An exciting opportunity has arisen for a Discharge Facilitator to work on Gastroenterology/Hepatology for a 6 month period to support the safe and timely discharges for our patients.
Ward 230 is a 36 bedded, diverse speciality both acute and fast paced, with a forever evolving environment.
You must have excellent communication skills as liaising with all members of the Multi-disciplinary team, nursing and medical colleagues and patients and their relatives is a key aspect of the role. You must have excellent computer and literacy skills.
In summary the role is
To assist qualified nurses to plan and facilitate a timely discharge process for patients from point of admission to the ward environment to the discharge of patients to their own home or alternative care facilities
To ensure that the Bed Management Tool is accurately
The main duties described in the job description will be undertaken under the supervision of a Registered Nurse within the defined ward areas.
