Integrated Discharge Team Facilitator

For full details about this varied and rewarding role, please see attached job description, but some of the responsibilities include To liaise effectively by establishing and maintaining professional working relationships with other members of the team, patients, carer, and relatives. This may include staff from other agencies or localities when a patients care is being transferred. Liaise with hospital and community based staff as appropriate to ensure continuity of care and assist in facilitating discharge in a timely manner To evaluate emergency situations and act accordingly working within procedures and report to the appropriate member of staff on duty. Maintain and update patients records showing a clear sequence of events and reasons for decisions taken in accordance with the Trusts guidelines To ensure personal targets/objectives are achieved and make a positive contribution to multi-disciplinary team, achieving projects goals/targets while supporting delivery of patient outcomes.
The post holder will undertake nursing/care tasks as delegated by qualified staff, within the competency scope of the individual. To improve the standard of patient care by utilising enhanced clinical skills to assess, plan, implement and evaluate care for patients.
