Patient Process Facilitators

Reporting to the Lead for Patient Flow and Discharge. The post holder will assist in the provision of an efficient, effective and comprehensive administrative service supporting the ward based team. The post holder will be expected to work alongside Patient Flow /Integrated Discharge Team as well as external partners to facilitate flow through the ward and discharge.
Using appropriate technologies and equipment the post holder will produce all work outputs to a high standard meeting the organizations requirements for Patient Safety, Information Governance and CQC recommendations in adherence with Trust policies, procedure and guidelines.
1. Support the Ward to assist patient flow and discharge.
2. Keep Patient Flow maintained and support electronic system update re patient’s location
3. Keep updated Patient Discharge Tracking List (OPTICA)
4. Attend ward huddles. Record actions and ensure that they are completed by the team in a timely manner
5. Schedule transport required for patient discharge
6. Access patient details using the trust electronic systems/Trakcare and SystmOne.
7. Input information onto active clinical notes.
8. Support the wards with paperwork required to discharge a patient
9. Assist in the process of patient flow through to ensure compliance with the Trust KPI’s
10. Support other clerical duties as required
11. Build strong communication skills with all members of the Multi Disciplinary Team ensuring that the most up to date patient information is available
12. Establish good working relationships throughout the hospital and with partners.
13. Deal with queries from service users including patients and their representatives in a manner that is consistent with legislation, policies and procedures whilst demonstrating understanding, empathy and discretion, taking accurate written messages and passing more complex queries to relevant staff members where appropriate.
14. Assist in planning and prioritising of workload to ensure that all service requirements and priorities are met
15. Data entry and retrieval of results/information where appropriate through the effective use of available technologies

We have an exciting opportunity available for Patient Flow Facilitators to join our team. You will be part of a busy team who respond to changes associated with patient flow requirements to ensure that capacity is available and support the patient journey. There is a combination of permanent and fixed term posts. Full and part time working will be considered.

You will be working as part of a well established Patient Flow Team/ Discharge Team and the wider multi disciplinary team to promote the efficient flow of patients through the ward.
You will support the transfer of patients out of the Trust and also be involved in the process to repatriate patients who require on going care within our Trust. You will work closely with internal and external service leads to ensure continuing improvement to the patients’ journey and successful return to the community.
This role will be flexible, working to cover as demand predicts over a 7 day period. We are looking for someone with strong organisational and communication skills and an excellent working knowledge of all IT systems used within a clinical health care setting.
