**Join Our Team: Dementia Care Lead & Activities Coordinator**
**The Oaks Care Home** is seeking a compassionate and creative individual to join our team as a **Dementia Care Lead & Activities Coordinator**. This dual role requires a unique blend of skills to provide both high-quality care and engaging activities for our residents with dementia. This is a new role to working Monday to Friday hours are 8:30am to 6pm, job shares are considered
**Key Responsibilities**:
- Deliver personalized care plans and provide support for daily living activities.
Engage with residents, friends and relatives to ensure care plans are person centred and recognise our residents diverse needs.
- Develop and implement a range of stimulating activities tailored to the interests and abilities of our residents.
- Foster a supportive and inclusive environment that encourages resident participation.
- Collaborate with healthcare professionals to ensure the best outcomes for residents.
- Maintain accurate records of care and activity engagement.
Lead dementia care across the home, leading training and development of the care team.
Wor collaboratively with the Catering team, Units Lead and carers to enhance the dining experience for residents
**Requirements**:
- NVQ Level 3 in Health & Social Care or equivalent.
- Experience in caring for individuals with dementia.
- Strong organizational skills and the ability to lead group activities.
- Excellent communication and interpersonal skills.
- A genuine passion for enhancing the lives of the elderly.
