Living Well Coordinator

Do you have excellent listening and communication skills? Are you organised with good administrative and IT skills? Do you enjoy working as part of a team? Do you want to help support West Northumberland residents to improve their health and wellbeing?
If you have answered yes to the above questions, then the Living Well Coordination Service would love to hear from you. We are recruiting one full-time Living Well Coordinator. This is an opportunity to be involved in delivering and supporting solutions for primary care patients across the West Northumberland Primary Care Network (PCN). The Living Well Coordinator will be the named worker for 1-3 GP practices and will split their working time across these practices, as well as working from Adapts office in Hexham.
You will also be required to work in other places throughout the North East, as identified in your individual work programme. The West Northumberland PCN practices are: Bellingham Practice, Branch End Surgery (Stocksfield), Burn Brae Medical Group (Hexham), Corbridge Medical Group, Haltwhistle Medical Group, Humshaugh and Wark Medical Group, Prudhoe Medical Group, Scots Gap Medical Group (Morpeth), Sele Medical Practice (Hexham) and White Medical Group (Ponteland). Adderlane Surgery (Prudhoe) and Riversdale Surgery (Prudhoe) are also supported by the service. Clients may live rurally beyond these areas.
