**About the role...**
We have an exciting opportunity for motivated Care Navigators to join our team based at Kings College Hospital UTC, on full-time or part-time, permanent basis.

**Your Day to Day**
The role of Care Navigator, based within our Urgent Treatment Centre, is to be patient focused, supporting UCC staff to communicate the appropriate use of urgent care pathways within the centre.
You will receive patient information following assessment from a GP or ECP/ENP, supporting patients to ensure they feel informed, supported and listened to so that they can make meaningful decisions and choices about their care.
You will support the UTC staff in collecting information and feedback from patients about their experience, and share this information with the CCG to improve community outreach and to achieve the key performance indicators.
**Qualification, Experience & Training**
- Educated to NVQ Level 2 or equivalent
- Working knowledge of Microsoft Office including Word, Excel and Outlook.
- Excellent interpersonal communication skills - able to pick up verbal and non-verbal clues about how people are feeling
- Knowledge of medical terminology
