**About this job**:
The Integrated Care Service, part of Adult Care and Support are looking for an experienced, confident, and committed individual to be part of our team. The successful applicant will have previously worked in Adults and Community Services or will have previous experience with working in the community to support adults who have a range of health and social care needs along with supporting their families. The role of Social Care Navigator requires Strength Based Approaches [“SBA”] in work with LBBD residents. The aim of this role is to help service users to see their strengths, maximise independence and choice and lead meaningful and fulfilling lives. You will undertake assessments and care reviews within the meaning of the Care Act 2014 and support social workers in a multidisciplinary setting. This is an opportunity to be part of a team of committed professionals with a strong social work vision and a commitment to improving the experiences of LBBD residents. The job involves weekend rota schedule.

This post is exempt from the Rehabilitation of Offenders Act 1974 and a comprehensive screening process will be undertaken on successful applicants including an Enhance Adults DBS Disclosure check.
