We are looking for a dynamic and motivated Consultant in diabetes to lead our community diabetes service across the Borough of Brent and Harrow. This post is in the region of 8 PA’s as a substantive post in a well-established Community Diabetes Service.

This is an exciting opportunity to join our friendly, multi-disciplinary diabetes team. The diabetes teams consist of GPs with special interest in diabetes, diabetes nurse specialists, diabetes nurse consultant, dieticians, podiatrists and diabetes educators. This is a role where you will be key in improving and developing our services.

We have close partnership working with primary and secondary care as well as local authority and the voluntary sector, and you will be integral to maintaining and developing this further.

We are in the process of developing our integrated pathways and working to align our services to the recently developed Northwest London Diabetes Specification.

The main purpose of this job is to provide strong clinical, professional and strategic leadership of the Diabetes services across Brent and Harrow.

The post holder will provide advice and support of the highest level for all colleagues and patients with diabetes in order to maintain clinical excellence, reduce the number of emergency admissions, emergency bed days and to promote patient self-care and quality of life managing complex patients.

The post holder will lead on Multidisciplinary team meetings both within a community and primary care setting and form close working relationships with the Acute services.

The post holder will work closely with clinical service leads and CBU managers to identify opportunities for service expansion and support the development and redesign of community-based diabetes services including pathway implementation.

The post holder will also provide advice and guidance to the Strategic & Business Development working closely with the system partners including GP's and commissioners.
