The Diabetes Care Co-ordinator is key to delivering an effective service to our children and young persons (CYP) with diabetes. This role provides efficient and high quality secretarial and administrative support to the paediatric diabetes service, working in close co-operation with staff, other health professionals, and patients and their families if appropriate. The postholder will manage the paediatric diabetes database, its data collection, and coordinate patient pathways to ensure achievement of Trust and national access targets and activity data collection.

The Diabetics Care Coordinator is responsible for the operational delivery of a range of functions associated with the paediatric diabetics MDT team and the service they provide.
- To be responsible for the timely capture of complex clinical and activity data from source documentation i.e. from medical notes and paper based case record forms, ensuring the data is accurately entered into the appropriate electronic data base.
- To be responsible for the reporting and presentation of Paediatric Diabetes data
- Ensure data is complete before submission internally and validated with stakeholders before external submission to commissioners and National Audits.
- To collect and enter sensitive patient data in to the database
- Extract, interpret and record information from patient notes as required by the established protocols.
- Ensure accuracy, completeness consistency and compliance with set national and trust targets.
- Act as first point of call for data query resolution for internal investigation.
- Liaise with consultants, Doctors, Nurses to resolve queries

St Georges Hospital has a long and proud history in providing specialist children’s services from the days of the Fountain Hospital. We currently provide an integrated mix of secondary care and specialist services to children of Wandsworth, Merton and South West London, Surrey and further afield.

Our aim is to provide an excellent, safe and innovative service that is child and family orientated. We are developing new services to help reduce inpatient admission while we work towards more consultant led provision of care.

We have a comprehensive range of paediatric specialist services in both medical and surgical fields. These are supported by PICU and NICU. Our medical specialities include: Allergy, Asthma, Cardiology, Endocrinology, Gastroenterology, Haematology, Infectious Diseases, Neurology, Epilepsy, Respiratory and Rheumatology with a joint treatment centre for Oncology with the Royal Marsden Hospital. These services offer a seamless transition from birth through to transition to adulthood.
