Management responsibilities To undertake delegated operational management tasks and deputise on behalf of the Manager and Service lead. Focus on advanced clinical practice in frailty with the intention to develop and take forward in your ACP role, also taking into consideration the needs of the service. To continually assess the needs of the service and work in collaboration with team and service managers to monitor service needs and ways to improve the service. Assist in the development and monitoring the quality of the Joint Emergency Team/ Urgent Community service and contribute to the development of quality improvement measures, including the full audit process.

To maintain accurate computerised and paper records in line with NMC/HCPC and Trust guidelines. Undertake investigation of complaints and near misses, taking follow-up action as appropriate To participate in the debriefing of staff following any incidents to co-operate with any investigation undertaken. To work within the policies and procedures outlined by the Oxleas NHS Foundation Trust and the NMC/ HCPC Codes of Professional Conduct. To ensure patient confidentiality.

To ensure own professional development through PDR process To participate in the promotion and development of patient information leaflets/booklets To undertake formal and informal supervision of junior staff and actively participate in own clinical supervision. Monitor and manage sickness and absence. To promote the local and national priorities of health care delivery e.g., Long Term Conditions and National Services Frameworks, Frailty. To support and manage social care colleagues in the service.

To have an overview of the commissioned rapid social care hours To utilise team resources effectively both health and social care. To monitor patient flows and needs of the service on each shift to identify increase in service demand and take appropriate action. To maintain Healthroster and ensure safe staffing levels, including the booking of bank and agency staff where necessary, monitor social care rota as above. Contribute in the development of a specialist service and act as a change agent and facilitate the change process whilst encouraging others to be innovative and adaptable in the approach to change.

Participate in the recruitment, selection, and retention of staff. To be conversant with all Trust policies and procedures and ensure they are correctly implemented. To represent the Trust on National Forums related to Advanced Clinical Practice. Adhere to confidentiality at all times with sensitive personal data.

Prioritise, organise, and manage own workload in a manner that maintains and promotes quality. Monitor the time management of junior staff and offer guidance and support. Maintain accurate records in relation to equipment used and maintained. Leadership To demonstrate and utilise a variety of leadership and influencing skills and be able to work with health and social care colleagues within integrated team and services.

To provide leadership and line management to delegated members of the team as required. To develop own leadership skills within the Team. Provide clinical supervision to junior team members. To implement change as identified.

To be a sign of mentor for student nurses/ AHPs To participate in clinical supervision and to use appropriate mentorship and support to other practitioners. To maintain own Clinical Professional Development To be able to delegate appropriately, ensuring that staff are competent to undertake and perform delegated duties and tasks To act as a mentor/preceptor to new staff, junior staff, students both health and social care and other non-qualified team members and support them to achieve their learning objectives. To participate in the Personal Development Review (PDR) process and in so doing identify training needs for self and delegated members of the team and encourage access to professional development opportunities. Develop evidence-based policies and procedures specifically required by the team.

Clinical To deliver and participate in clinical care of patients. Provide a level of advanced and comprehensive assessment, based on a specialist body of knowledge. Demonstrates the advanced skills and knowledge to undertake comprehensive person-centred history taking, assessment and formulation, considering biological, psychological, social, and spiritual perspectives. Direct responsibility for the management of patients within own case load and for the supervision of junior members of the multidisciplinary team To appropriately assess, examine, investigate, diagnose, and treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations.

Order and interpret investigations, laboratory tests and act on results as appropriate. Initiate drug therapy/medication within the parameters of agreed clinical guidelines and in accordance with present legisl
