Aspects Care Ltd is currently looking for a full time experienced Care Coordinator, to assist the Registered Care Manager with the day to day running of the service. The position is contracted to work 40 hours per week over 7 days, including weekends and on call duties.

You will work with our Service Users who have complex learning disabilities, Challenging behaviours and mental health, supporting them to live independently in their local community under a supported living programme. You will be required to work within a Person Centred framework ensuring that care plans reflect the needs of the individuals.

You will be responsible for managing service user staff teams, carry out supervisions, oversee rota management, conduct project spot checks, and visits to the service user homes. You will also attend and actively participate in regular staff/team meetings whilst developing and maintaining relationships with external professionals. You will be accountable for ensuring all paperwork is completed in line with CQC and company guidelines, seeking out opportunities to improve

The post is office based but on call duties are required. Applicants must be educated to NVQ level 3 in Health and Social Care or equivalent and must have experience in a similar role working with service users with challenging needs. Applicants must have a full driving licence and access to a car with business insurance.

Successful applicants are required to provide an enhanced disclosure. Disclosure expense will be met by applicant. Salary starts from £23,000 to £26,000 per annum dependent on qualifications and experience.
