The purpose of this post will be to work with the programme director and programme managers within an established health and care partnership to contribute to the delivery of a far reaching change programme across mental health services, adult social care and the voluntary sector.

The programme will have a particular focus on access and demand. You will work to improve the delivery of operational services, processes and systems. Supporting and involving operational managers in the delivery of a transformation programme that improves the performance against key system metrics and outcomes for people who use mental health/ adult social care services.

The role will take a lead role on developing robust processes and procedures and ensure that information on good practice and organisational learning is widely available. It will also take a lead role in improving the quality of the service and delivering agreed operational improvements in the department and with partner agencies and other stakeholders to secure the continuous development, improvement, efficiency and success within areas of adult social care and mental health services

Brent Integrated Care Partnership (Brent ICP) brings together health and care organisations from across the borough. It aims to work collaboratively with all the health, care and wellbeing organisations that serve the community of Brent.

The ICP is dedicated to improving the health and wellbeing of residents and is committed to working with all of our diverse communities to ensure everyone’s needs are met.

We aim to place the resident at the heart of health and care services in order to tackle health inequalities together - working as equal partners and with a collective goal to improve the wellbeing of everyone in Brent.

Because we want the roles to be available to people from a range of different professional backgrounds, these roles are being advertised in CNWL and at Brent Council. The roles will be based at Brent Civic Centre in Wembley, and will involve 3 days working in the office per week, with home working available for the other days.

MAIN DUTIES AND RESPONSIBILITIES

Service Improvement

Lead and take accountability for the delivery of projects related to service improvement within mental health and adult social care; this will include for example service specific areas of focus such as access and demand, communications relating to mental health awareness, promotion of Talking Therapies and delivery of the Brent business case for rollout into NW10 and NW2 postcodes.

To embed workforce change into all improvement initiatives, supporting managers to identify and source appropriate OD & LD interventions.

Oversee the production of programme documentation, including programme plans and reporting templates, including documentation for relevant committees and working groups.

Lead research into additional external sources of funding to support the transformation objectives aligned to the programme

Develop and implement practical ways of improving the quality and speed of communications from the programme to providers, staff and wider partners.

Ensure that methods of measurement and KPIs for the programme are established and maintained, and to contribute to the wider system-level dashboard development work being undertaken by the Head of Health Evidence and Insight.
