Patient Safety Incident Investigator

An exciting opportunity has arisen for a new post, Patient Safety Incident Investigator to join the Risk and Patient Safety Team. Part of a small team of investigators you will need to be self-driven with excellent communication and interpersonal skills with an ability to work at all levels of the organisation. You will take pride in your work, be highly motivated, professional, have a flexible approach, good organisational skills, have strong professional curiosity, excellent attention to detail and be able to work to deadlines.
If you are passionate about patient safety, and have the ability to identify and review all sources of information in relation to patient safety incidents using a variety of analytical skills to identify systems learning and a desire to work closely with and support patients, service users, carers, their families and staff throughout response processes, encouraging and supporting active engagement, this could be the role for you.
The Risk and Patient Safety team is a small, friendly, supportive, and effective corporate function which oversees and manages the following areas: Safety Incident Reporting, Patient Safety, Risk Management, Learning from Deaths, Quality Governance and compliance. The whole team works in a hybrid and flexible way to support the needs of the organization and individual members. As a Patient Safety Incident Investigator, you will support the Head of Patient Safety to provide Trust-wide oversight, assuring quality and effectiveness of all types of patient safety investigations as part of the Trust’s annually agreed Patient Safety Incident Response Plan (PSIRP). This is required within the Patient Safety Incident Response Framework (PSIRF) which has been implemented in response the NHS Patient Safety Strategy (2019).
The Patient Safety Incident Investigator will undertake robust patient safety incident investigations by working with a range of internal and external stakeholders to investigate, analyse and use judgement through the collation of evidence and findings using the agreed methods within the Trust’s Patient Safety Investigation Plan (PSIRP). The outcome will be the identification of systems learning from a patient safety incident that will then guide sustainable risk reduction and service improvement, ensuring that a Just and Learning Culture and human factors principles are maintained. The Patient Safety Incident Investigator will provide visible senior leadership and be a role model to engage staff, patients and carers and enable the delivery of improved quality of care and patient and staff safety.

- Undertake Duty of Candour in a timely and sensitive manner when required with patients and families and educate and support clinical colleagues in these conversations.
- To ensure that all patient safety incident investigations (PSII) undertaken are logged on the Datix risk management system and all documents pertaining to them are saved to the relevant record.
