The Older Adults Psychiatric liaison team require a fulltime band 3 clinical support worker to join the team.

The Liaison team operates over 7 days a week but the clinical support worker will be expected to work Monday to Friday 9am to 5pm.

**The team provide**:
liaison assessments throughout the hospitals/sites of the local acute trust

we provide advise and suggestion interventions, treatments and work alongside of acute colleagues towards improving the mental wellbeing of patients.

we provide education to acute staff and cares, family and to the patients about mental health.

we work closely with families and other services such as social care, physiotherapy and occupational therapy.

In Liaison psychiatry, staff undertake comprehensive biopsychosocial mental health assessments alongside robust risk and safety planning assessments.

When patients remain in the acute hospital for medical treatment, we provide liaison interventions which may include brief psychological interventions.

Liaising with professional's involved in the person's care including family and carers, acute care staff, social care and other CNTW services.

To support the liaison interventions for patients receiving acute medical treatment.

Be part of reviewing care and treatment provided by liaison staff.

Provide person centred education to acute staff by modelling interventions.

Provide feedback to liaison MDT - written or verbal following support worker review of liaison patients.

To work alongside of carers, family member's and acute colleagues in acute hospital.
