The purpose of iCAN programme is to deliver a refreshed focus and way to improve the quality of care and achieve the best possible health and wellbeing outcomes for older people across our county, supporting them to maintain their independence and resilience for as long as possible. The three core aims of the iCAN programme are to:

- Ensure we choose well: no one is in hospital without a need to be there
- Ensure people can stay well
- Ensure people can love: by staying at home if that is right for them

In this role you will be employed as an Assessment and Enablement Worker (AEW) in the Wellingborough Adult Social Care Hub but will also work closely with the frailty clinics and be part of the welfare support team operating under the iCAN delivery programme. Within this role you will have the opportunity to work jointly with all relevant partner organisations involved in the programme.

You will be carrying out welfare calls to people within PCN as well as A&E discharges. Within these initial calls, identify their needs and follow up as appropriate. Undertake health checks such as weight, nutrition, blood pressure monitoring, temperature and personal care needs. You will be supporting people within the community to be able to remain as independent as possible within their own home and reduce the risk of crisis support. Tasks may include but not limited to assessment and provision of low level mobility aids, assisting with home management tasks, provision of items to meet basic needs such as emergency food parcels, supporting individuals with housing, finances, medication and follow up appointments. Within the role you will also coordinate the frailty clinics, identify people who would benefit from this clinic, facilitate appointments within people’s own home and ensure they are supported throughout the process.

You will have the opportunity receive training from both ASC and Health and you will also work across both systems.
