Hospice at Home Team Lead

This an exciting opportunity to join the Community Specialist Palliative Care Team as a Team Lead. The Team Leam will be required to influence patient care by participating:
- Leadership of the team and service development
- High level of clinical practice
- Coordination and triage of referrals into Hospice at Home
- Training and education
- Research and audit
You will be leading a team with a high level of clinical practice who are responding to the physical, psychological, social and spiritual needs of patients and those who are important to the patient; ensuring the assessment and management of palliative care needs. There is an expectation to ensure care reflects the strategic priorities of NHFT; ensuring a flexible, responsive service that meets the local needs of people across Northamptonshire, whilst upholding Care Quality Commission requirements
The band 6 post holder will be based at Cynthia Spencer Hospice, although there will be a requirement to travel across County; based on service need. This is a full-time position of 37.5 hours.
Previous experience in palliative care and leadership is essential for this post.
Excellent communication skills will be required and an ability to support people with complex emotional and psychological needs.
Car owner / driver and current driving license essential or access to a car (unless you have a disability as defined by the Equality Act 2010).

We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
