Out of Hours Clinical Co-ordinator

An opportunity has become available to join our Out of Hours Clinical Co-ordinator Team. This service has now been running for several years and has been identified as a key service to address the reduced number of senior clinicians available out of hours.
The role will work across a wide range of services including acute inpatient, PCDU, CRHT’s, Older adults, CAMHS to provide professional support and advice and to also be a point of contact for external agencies such as ULHT, police, emergency duty team and the LPFT on-call manager and executive on-call.
A key purpose of the service is to provide a senior onsite manager to support clinical staff.
To coordinate out of hours admissions, both in county and out of county
Work with in hours managers to ensure staffing levels are appropriate across clinical teams with the staffing resource being used most effectively.
To support services with out of hours clinical leadership and advice on a wide range of issues, such as bed management, staffing, admission reduction and to ensure that delivery of effective patient care continues during the night working hours and weekends. This is a role that will cover a 365 day per year support service across trust wide inpatient wards.
The post holder will act as Bronze Command for LPFT.
To ensure that a senior clinician is available 24/7 to provide clinical leadership and decision making.
To ensure cost effective use of resources and ensuring admissions are appropriate and purposeful

Provide senior leadership & provide Bronze Command/ delegated authority to act
Act as the senior clinician out of hours to manage highly complex and challenging clinical scenarios and provide clinical support to aid decision making
Support all wards in LPFT & manage staffing short falls in out of hours. Co-ordinate movement of staff if required
Support the admission to discharge process across the wards to facilitate bed flow
Act as a point of liaison with ULHT and LCC out of hours for routine issues
Oversee the acuity and occupancy of the wards during the night & weekend period
Ensure ALL bed capacity within the division is explored before consideration given to Out Of Area (OOA)
Receive the OOA request form and authorise if appropriate
Allocate in county beds, arrange transfers and manage acquisition of out of county beds should they be required
Manage and triage need around secure patient transport
To receive information about any patient incidents e.g. absconded, assaults
Act as a point of liaison with on call consultants
Oversee the acuity and occupancy of the wards during the night & weekend period
Support the handover process of the most challenging patients between shifts and offer support and advice in the management of those with complex presentations
Manage the Section 140 bed requests
This person will be the only person who has the authority to call the On Call Manager if they are unable to resolve issues locally
