**A Patient Care Co-ordinator required for a busy GP practice for 25 hours a week, on a permanent basis.**

Ifield medical Practice is a GP practice in Ifield, Crawley. We have 4 GP partners and 1 Salaried GP. We have a wonderful team of Reception, Admin, and Medical Secretaries, and we are looking for a new Care Co-ordinator to provide further support to our patients and the practice following an internal promotion.

**Job Summary**:
This job is to support the smooth co-ordination of patient care for the benefit of our patients.

The Care Co-ordinator will be responsible for consulting with patients to help them access the support they need, which may include liaising with other health and/ or social care services.

**Main duties include**:

- Signposting patients appropriately to outside agencies who can best support their needs.
- Liaising with patients to help identify social needs, carrying out appropriate referrals to community teams, social prescribing teams, and social services for social support.
- Actively listening to our patients with empathy and respect, and providing personalised support.
- Provide support to informal carers
- Attend MDT meetings on a monthly basis, and other relevant meetings
- Provide continuous care for patients identified on a regular basis, to ensure they are engaging with services
- Keep accurate, up-to-date records of contacted patients, carers, and professionals, using EMIS.
- Maintain patient confidentiality and quality care.
- Complete and maintain care plans for patients with complex needs, as well as being a main point of contact for the practice for these patients.
- Understanding when it is appropriate to refer patients back to health professionals (the GP) when the person’s needs is beyond the scope of the care co-ordinator

- Actively listen and empathise with our patients
- Communicate effectively (both verbally and in writing) with people, their families, carers, community groups, and partner agencies.
- Maintain effective working relationships.
- Work flexibly and work well under-pressure
- Plan and prioritise own initiative and workload.
- Accurately record and update medical records, as required.

**Personal qualities and experience -**
- Kind, caring, and helpful manner
- Good IT skills
- Self-motivated
- Experience with working in a health / social care environment is essential
- Experience with working with EMIS (is desirable however training can be provided)
- Meets DBS standards and criminal record checks

**Hours**:
Monday 08:00 - 1pm

Tuesday 08:00 - 1pm

Wednesday 1pm - 6pm

Thursday 08:00 - 1pm

Friday 1pm - 6pm
- There is some flexibility on the hours to total 25 hours a week over 5 days._

**Job Types**: Part-time, Permanent
Part-time hours: 25 per week

**Salary**: £11.00 per hour
