Complex Case Practitioner

We are seeking a dedicated Complex Case Practitioner to join our team. As a Complex Case Practitioner, you will be responsible for providing comprehensive support and assistance to individuals and families facing complex challenges. Your role will involve assessing needs, developing care plans, and coordinating services to ensure the best possible outcomes for our clients.
**Please Note: This role includes some evening and weekend on-call duties (rotational basis) and occasional travel to support families.**
**In your new role you will**
- Promote GMT Core Values and DSR Crisis Response Program.
- To action all risk assessment strategies with regards to individual children and young people.
- To implement the Missing From Home protocol as required.
- To support with the co-ordination of services and attend all MDT and Care Planning / PPM meetings.
- To contribute to a positive child friendly atmosphere in the home.
- To be alert and observant with regards to the well-being of the young person and their family.
- To be part of the staff team and to participate in training.
- To attend regular supervision and always be aware of your own resilience levels.
**What do you need to succeed in your new role?**
- Educated to a good standard
- Level 3 Diploma for the Children and Young People’s Workforce
- Recognised Social Work Qualification
- Relevant Education or Childcare Qualification
- PMVA trained
**Essential Must-Haves**
- Listening and counselling skills.
- Trauma informed.
- To have an understanding of the Biopsychosocial approach.
- Ability to handle conflict.
- Ability to use initiative.
- Ability to work as part of a team.
- Oral and written communication skills.
- Ability to have good organisational and time management skills.
- Ability to keep records and write reports to a high standard.
- Ability to engage in good child care practice.
- Ability to support children constructively.
- Ability to look at situations from all perspectives and support with mediation.
**Embracing the Unexpected**
Due to the unpredictable and varied nature of Great Minds Together's work, staff should be flexible, willing to work weekends and evenings as needed. Additional duties, usually compatible with regular tasks, may arise to cover unforeseen circumstances or changes. If these tasks become regular, they'll be added to the job description after consultation with the staff member.
